Annexure to Draft Consent Parenting Order

Each party to the application for parenting orders or his/her lawyer must complete a separate annexure. Please refer to Consent Orders Supplement for use in parenting matters where proceedings have already been filed in the Family Court (Do it yourself kit).

Staple the annexures to the back of the original of the proposed consent orders signed by the parties.

Part B must be completed by the person who signs the annexure.

Part C is only completed where there have been allegations of abuse.
