Hi,
I know they're here somewhere but I can't find them for the life of me !!!!
Some kind soul posted an attachment which had examples of Questionnaire, Statement of Issues and a Chronology.
I have 'searched'.....
I have tried to remember the name of the member....
I am now getting tetchy.....
If anyone has the slightest clue what I am talking about could they help ?
And could they be put in the Download section ? Or if that is where they are, could they just be easier to see ?

If anyone has time....
It would be really really helpful to LIPs, if a link to these docs could be put as a sticky at the front of the Self Representation Forum, perhaps alongside "How to complete a Form E"
