Forms apparently related to mediation 6 Months, 1 Week ago

Please could someone tell me what an "APP7" and a "Form FM1" are? I have been referred to the mediation company FMM Ltd., and they state that they issue these forms at the end of mediation or if mediation is unsuitable. Currently I am in the position of wondering whether to accept the offered mediation or not and am wondering what will happen if I don't accept it (the mediator felt that I might not be "ready" for it).

In April the rules changed so that people who wanted court to help have to try or at least be assessed for mediation (MIAM) if this is refused or fails the mediator give you FM1 which is needed before an application can be made to court.
