Applying for a 'Declaration of Parentage'at the county court. I have to file a sworn statement or 'Affidavit' with it.

Does anyone know how to write a basic one.

I think I replied to your question about Form FL 423 on another forum earlier today.

You need to state any information or belief that supports your application that you are the parent and quote the sources of your information or belief. Then declare a statement of truth - "I believe that the facts stated are true."

I don't think it needs to be signed in the presence of a person who is authorised to administer oaths but you can check that with court officials because living in Scotland where procedures are different I'm not sure now.

If the officials don't know ask them to check the Family Proceedings Courts (Family Law Act 1986) Rules 2001

Its the format that worries me - i am ok with stating the facts it was just how to word things.

Use the attached doc which uses applicant and respondant instead (you would be applicant in this matter)

You need to put the case number and the court that is hearing it at the top of the paper, then your name as the claimant and the respondents name i presume your ex? then write affidavit of (your name) and the date. start with an introduction as to what you are seeking and set out why you are seeking it. Try to keep it factual and unemotional. at the end you have to have a statement of truth where you simply say, I believe this information to be the whole truth. good luck
