Help with form D80 & D84

Hello, you've already helped me complete the D8. Now got the next forms and would appreciate a bit more advice.

Form D80 Affadavit by petitioner
Q5 State briefly the reason for the separation - in my D8 stated the marriage has irretrievably broken down - would that do?

Q6 State the date when and circumstances in which you came to the conclusion that the marriage was in fact at an end - can I write the same date as Q5 and when the respondent and I agreed to live apart?

Q8 Since the date given in Q4 have you ever lived with the respondent in the same household? - my answer is Yes we lived separately in the same household until the house was sold - is that ok?

Form D84 Application for direction for trial state The petitioner (do I write my name here) applies to the District Judges for direction

phew! Long post but really hope someone can help as doing it solo thanks ever so

1. usually write something slightly different - e.g. there had been problems for some time and we agreed that the marraige had broken down / I learned that the Respodent had committed adultery
2. yes, same date & saying you agreed to live separtely is fine
3. Yes, however you will also need to put a little more info abotut how you were living separately, e.g. that you had separte bedrooms, did not cook / clean for one another.
4.yes, put your name in
